What is the cause of infertility? Man's reason and woman's reason
Etiology of infertility
Both men and women should consider that as long as one factor in the pregnancy conditions is abnormal, it can hinder pregnancy.
Man's reason
The cause of infertility in men accounts for about 40%. ① Affect spermatogenesis: such as congenital testicular dysplasia or cryptorchidism, no spermatogenesis, or acquired inflammation, such as tuberculous orchitis or varicocele, affect spermatogenesis. ② Affect sperm transport: epididymal diseases or congenital vas deferens obstruction affect sperm excretion. ③ Affect semen entering the cervical canal: if the penis is too short, hypospadias, premature ejaculation, impotence, etc., it can affect sexual intercourse. Semen cannot be injected into the vagina and reach the cervix. ④ Abnormal semen: such as dead semen, sperm incompetence, semen thickening, etc.
Woman's reason
The causes of infertility in women account for about 45%. ① Affect egg development: hypothalamus pituitary ovary axis dysfunction, affect egg maturation or interfere with egg excretion; Congenital ovarian hypoplasia, surgical ovariectomy or changes in the external environment and mental and emotional effects can affect ovulation. ② Affect the delivery of eggs, sperm or fertilized eggs: after the eggs are discharged, they enter the fallopian tube. After the sperm is injected into the vagina, they also meet the eggs through the uterine cavity and fallopian tube, and the fertilized eggs also enter the uterine cavity through the fallopian tube. The study found that many infertile women with reproductive tract inflammation have a kind of bacterioplast in their vagina, cervical secretion and endometrium, which may be the cause of infertility. It is estimated that it is related to the attachment of bacterioplast to sperm tail and affecting sperm activity. In the unexplained infertile women, the rate of bacterial physical examination was as high as 85 ~ 95%, while that in the control group was 23%. After treatment, the pregnancy rate can be significantly improved. ③ Affect sperm ascending: such as vaginal diaphragm, vaginal cervicitis, uterine mouth atresia or endocrine imbalance, resulting in excessive cervical mucus, etc.
Gender factors
About 15%, mainly immune infertility. Sperm, eggs, fertilized eggs, gonadotropins, sex hormones and secretions in the whole process of pregnancy have certain antigenicity, which can cause immune response and infertility. This immune response can be divided into two categories. One is alloimmunization, which refers to the protein or exfoliated cells secreted by men's sperm, seminal plasma and reproductive tract, or the female immune response caused by fertilized eggs to produce corresponding antibody substances against it. The other is autoimmunity, which means that the sperm and seminal plasma of the man or the egg and reproductive tract secretion of the woman enter the surrounding tissues, resulting in autoimmune reaction, produce corresponding antibody substances and affect the vitality of their own sperm or eggs.
Other factors
Such as malnutrition, metabolic disorders, chromosome abnormalities or physical and mental factors can cause infertility.
